From Grant Applicants:

  • Patient and guardian names, contact information, and date of birth
  • Area of medical concern, disability status, and care setting
  • Physician or therapist contact details
  • Financial information (e.g., income range, optional W-2 or tax return)
  • Grant request details, including denial letters and estimated costs
  • Optional letters of support from medical providers
  • Media release consent and typed signature

From Donors, Volunteers, and Website Visitors:

  • Name, email, phone number, and mailing address
  • Donation history and preferences
  • Volunteer interests and availability
  • Social media interactions and submitted stories or photos

How We Use Your Information

We use personal information solely for the purpose for which it was provided, including:

  • Reviewing and processing grant applications
  • Communicating with applicants, donors, and volunteers
